Cover Letter A cover letter is required. Cover letter must describe the significance and uniqueness of the work and note any prior correspondence with the journal and any prior conference presentation, if applicable.
Abstract An abstract is required. Abstract must not exceed 275 words. Abstract must be structured. Abstract must use the headings PURPOSE, PATIENTS AND METHODS (or MATERIALS AND METHODS, METHODS, or a similar heading), RESULTS, and CONCLUSION.
Manuscript Manuscript must not exceed 4000 words, although initial submission may exceed this limit by up to 1000 words.
References References are limited to 150. References must be numbered in the order in which they are cited in the text.
Figures & Tables Figures and tables are limited to 6 combined. Tables must be placed at the end of the manuscript, and figures may be embedded in the manuscript instead of uploaded separately. Figure legends must be included in the manuscript file, and tables must be cited in numerical order using Arabic numerals.

Abstract A structured abstract is required. Abstract must be 275 words. Abstract must be structured. Abstract must use the headings PURPOSE, PATIENTS AND METHODS or a similar methods heading, RESULTS, and CONCLUSION.
Manuscript Manuscript must not exceed 3000 words, although initial submission may exceed this limit by up to 1000 words. Manuscript must include a Context Summary following the title page at revision with Key objective, Knowledge generated, and Relevance sections. At revision, the manuscript must include a Context Summary of fewer than 100 words in which Key objective is a single question defining the content and novelty, Knowledge generated consists of two short result sentences without conclusions or interpretation, and Relevance explains clinical application.
References References must be numbered in the order in which they are cited in the text.
Figures & Tables Original Reports are limited to 6 figures and tables combined. Tables must be placed at the end of the manuscript, and figures may be embedded in the manuscript instead of uploaded separately. Figure legends must be included in the manuscript file, and extended patient demographic data should be presented in a table rather than in the text.

Review Articles

Abstract An abstract is required. Abstract must be 275 words. Systematic reviews, consensus guidelines, and meta-analyses must use a structured abstract, while other Review Articles must use an unstructured abstract. Structured abstracts must use the headings PURPOSE, PATIENTS AND METHODS, RESULTS, and CONCLUSION, with DESIGN permitted instead of PATIENTS AND METHODS for Review Articles.
Manuscript Manuscript must not exceed 4000 words, although initial submission may exceed this limit by no more than 1,000 words. Review Articles should be disease-focused, examine new data in the field, and describe applications to practice. Review Articles may include one optional graphical abstract at revision, submitted separately, with text under 100 words and 1 to 3 figures.
References References are limited to 150. References must be numbered in the order in which they are cited in the text.
Figures & Tables Review Articles are limited to 6 figures and tables combined. Tables must be placed at the end of the manuscript, and figures may be embedded in the manuscript rather than uploaded separately if included there. Abstract Abstract is not required for Case Reports.
Manuscript Case Reports must not exceed 1500 words, although initial submission may exceed this limit by no more than 1000 words. Case Reports should provide a brief overview of the case and a concise literature review. Case Reports must be supported by data, should provide access to primary data, and should include clear evidence of exceptional response or resistance, a proposed mechanism linking genomic features to response or resistance to targeted therapy, and the variant, platform, and targeted therapy used.
References Case Reports are limited to 20 references. References must be numbered in the order in which they are cited in the text.
Figures & Tables Case Reports are limited to 3 figures and tables combined. If figures are included within the manuscript, they do not need to be uploaded separately, and tables must be placed at the end of the manuscript. Figure legends must be included in the manuscript file, and tables must be cited in the order in which they appear in the text using Arabic numerals.

Clinical Trial Updates

Clinical Trial Updates should include a brief description of the trial and a short summary of previous findings to provide context. Updates to randomized phase II and all phase III trials must include a redacted study protocol.
