Original Investigation

Cover Letter A cover letter is required. The cover letter must state whether the authors have published, posted, or submitted any related papers from the same study.
Abstract A structured abstract is required. The abstract must be structured.
Manuscript Text must not exceed 3000 words, excluding the abstract, tables, figures, acknowledgments, references, and online-only material. The manuscript file must include a title page, abstract, text, references, and, as appropriate, figure legends and tables. The manuscript must include 3 Key Points and a Data Sharing Statement.
References References must follow AMA style. References must be numbered in the order of appearance, cited with superscript arabic numerals, not alphabetized, list up to 6 authors or editors, use the first 3 followed by "et al." if more than 6, abbreviate journal names according to PubMed, and include the issue number after the volume number for journal references.
Figures & Tables No more than 5 tables and/or figures are permitted. Figures must be submitted as separate files, while tables must be included at the end of the manuscript file and not uploaded as separate files. Each table and figure must be numbered, cited in the text, and limited to material needed to support the article, and primary outcome data must not be presented only in figures.

Systematic Review (Without Meta-Analysis)

Abstract A structured abstract is required. Abstract must be no more than 350 words. Abstract must be structured. Abstract must use the headings Importance, Objective, Evidence Review, Findings, and Conclusions and Relevance.

Manuscript must include Abstract, Introduction, Methods, Results, Discussion, and Conclusions, and the subtitle should be "A Systematic Review." Key Points of 75 to 100 words are required and must use the headings Question, Findings, and Meaning.
References Manuscript must include 50 to 75 references. References must follow AMA style. References must be numbered in order of appearance using superscript Arabic numerals, not alphabetized, list up to 6 authors or editors, use the first 3 followed by "et al." if more than 6, abbreviate journal names according to PubMed, and include the issue number in journal references.
Figures & Tables No more than 5 tables and/or figures are permitted. Figures must be submitted as separate files rather than included in the manuscript text, while tables must be included at the end of the manuscript file and not uploaded as separate files. Include a table with ratings of study or evidence quality, construct tables summarizing search results, and treatment summary tables should include treatment name, strength of evidence, treatment effect preferably with 95% confidence intervals, adverse effects, and very brief comments if necessary.

Narrative Review

Abstract A 3-part structured abstract is required. Abstract must not exceed 300 words. Abstract must be structured in 3 parts. Abstract must use the headings Importance, Observations, and Conclusions and Relevance.
Manuscript Manuscript length is typically 2000 to 3500 words maximum. Manuscript should include Abstract, Introduction, Methods if included, Discussion/Observations, and Conclusions, with the subtitle "A Review."

For most Narrative Reviews, tables should summarize epidemiology, diagnostic tools, and available therapies, and a fourth table should compare the review findings with current clinical practice recommendations and/or diagnostic and therapeutic uncertainty and controversies.

Clinical Trial

Abstract A structured abstract is required. The abstract must be structured and end with trial registration information including the registry name, trial ID, and URL.
Manuscript The manuscript text must not exceed 3000 words, excluding the abstract, tables, figures, acknowledgments, references, and supplemental material. The manuscript must clearly state an objective or hypothesis, design and methods, essential features of interventions, primary and secondary outcome measures consistent with the trial protocol, main results, discussion of study limitations, and conclusions. The manuscript must include 3 Key Points, a Data Sharing Statement, a power or sample size calculation statement, and for randomized trials the subtitle phrase "A Randomized Clinical Trial" or for nonrandomized clinical trials "A Nonrandomized Clinical Trial."
References The manuscript must include 50 to 75 references. References must follow AMA style. References must be numbered in order of appearance using superscript Arabic numerals, not alphabetized, list up to 6 authors or editors, use the first 3 followed by "et al" if more than 6, abbreviate journal names according to PubMed, and include the issue number in journal citations.
